Select one or more Certifications and/or Speciality Programs and add them to your application using the "Enroll Now" button. Once you are finished, complete your application using a major credit card and within the week you will receive a copy of the NFPT Study & Reference Materials and/or CD-ROM.

You can either print and complete our printable enrollment form for immediate mailing. Or, simply complete and send in the hard-copy enrollment form you received with your brochure through the mail if you made a previous brochure request. Then, enclose the appropriate fees, and send them to NFPT Headquarters. Your enrollment form will be processed and you will be sent the NFPT Study & Reference Materials.

Use your fax machine to apply. Download and complete the printable enrollment form or enrollment form you received in the mail including credit card information. Then, simply fax the form to our headquarters-fax #(765)471-7369. If you do not have a fax machine, most printing companies can send a fax for you. Your enrollment will be processed and NFPT Materials sent to you that very day.
